All WorkCover and TAC claims will require a GP referral and documents showing the claim has been accepted and stating a valid claim number.

EPC/CDM plans can be provided by your GP if you meet certain chronic health criteria. EPC/CDM plans provide up to five sessions per year of medicare subsidised Physio. Please note Evolved Physio does not bulk bill. Clients with EPC plans will be charged normal rates and be eligible to claim back the Medicare subsidy (currently $60.30) onsite at the time of consultation.
